How do I obtain a copy of my medical records? |
| A. |
You can obtain a copy of your medical records by requesting them in writing from the hospital´s Health Information Management Department. If your physician is a member of the medical staff, they can make the request for you. Your personal representative can also request your records provided they have proper legal documentation of their authority. |
