Q. How do I obtain a copy of my medical records?
A. You can obtain a copy of your medical records by requesting them in writing from the hospital´s Health Information Management Department. If your physician is a member of the medical staff, they can make the request for you. Your personal representative can also request your records provided they have proper legal documentation of their authority.
Q. How do I obtain a copy of my child´s birth certificate?
A. To obtain a copy of your child´s birth certificate, contact the Maryland Department of Health and Mental Hygiene´s vital records office at 410-764-3038, or 410-764-3036.
